Standard dental cleanings, all x-rays and two yearly exams
All other dental services such as basic, major, endodontics and periodontics will be provided at a 20% discount (20% Discount excludes orthodontic treatment or products)
$350.00 for Patients 12 and over
$250.00 Kids Plan (children 11 and under)
$400.00 for Patients 12 and over
$450.00 for patients that need a periodontal plan including a 20% discount on scaling and root planning and three perio maintenance cleanings a year
Payment in full must be made at the time the services are rendered to be eligible for the discount. Membership will be voided if full payment is not satisfied. This is a dental discount plan and is NOT dental insurance — it cannot be combined with any other dental insurance.
This plan is only valid at Folly Beach Family Dentistry. Therefore, if you are referred to a specialist, they will not offer this discount. Should there be dental treatment needed following any type of injury where an outside medical policy, care disability or workman’s compensation type insurance is involved, this discount plan can not be used. The plan is non-refundable and non-transferable between family members.
